Requirements: Registered with Singapore Nursing Board Has 2 to 3 years of nursing experience in chronic disease management A Specialist Diploma in Diabetes Education/Gerontology or Advanced Diploma in Chronic Disease Management is preferred A good team player Possess excellent interpersonal and communications skills Proactive and self-motivated Working location: Geylang / Kallang Polyclinic .. Candidates with relevant experience could be considered for Senior role.
