Responsibilities
\xc2\xb7 Provide coordination and care management for assigned patients
\xc2\xb7 Connect patient with community resource and financial assistance
\xc2\xb7 Perform nursing procedures in home environment
\xc2\xb7 Develop and evaluate care plan and goals
\xc2\xb7 Liaise with other healthcare partners who are caring for the same patient
\xc2\xb7 Participate in multidisciplinary round
\xc2\xb7 Maintain timely and accurate documentation of patients\' condition and care given
\xc2\xb7 Provide care giver training programme and health education to patients / caregivers and evaluate the teaching programmes to meet patients and caregivers needs and expectations.
\xc2\xb7 Participate in quality improvement project and nursing research
Job Requirements
- Registered Nurse with Singapore Nursing Board
- Minimum 3 years\' experience working in acute hospital settings and or familiar with Community hospital settings
- Candidates with Advance Diploma in nursing specialties preferred
- Able to be work on extended hours (5pm to 9pm) on certain weekdays, on rotational basis,estimates 3 to 4 times a month
- Must be comfortable to travel around on a daily basis
- Head office: Toa payoh
- Monday to Friday
