Job Description

:

• Comply with approved Nursing SOPs
• Responsible for assessing the needs and delivery of quality geriatric /dementia nursing care to clients
• Perform nursing assessments of Clients during enrolment
• Develop ICP for Clients and conduct case discussion with the other MDT members
• Provide caregiver education and training
• Establish patient centered approach
• Provide medication management support as needed
• Oversee Clients\' medical and nursing needs
• Maintain documentation of the Clients\' progress and outcome of care provision. Initiate Quality Improvement care as needed.
• Identify symptoms of clients related to illness or decline in functional/cognitive abilities and advocate early attention.
• Administer medications, treatments, and procedures as prescribed
• Conduct Home visits required for all Home Care provision (care assessment, nursing cares and environment check etc.)
• Enhance and increase the overall efficiency, as well as adherence to organization policy and procedures.
• Collaborate with various stakeholders and other professionals such as AIC, community nurses, RHS with social workers for the holistic management of clients.
• Collaboration between physicians and pharmacists
• Oversee adequacy of medical supplies/ equipment and ensure regular quality control check of medical equipment.
• Participate and ensure all requirements are met for Centre Audits.
• Oversee and ensure smooth daily operations in the centre.
• Plan monthly staff roster for Centre Manager\'s approval.
• Any other new project or duties assignment by Centre Manager

Qualification & Skills:

• Diploma / degree in nursing
• Registered with SNB
• Experience working in an elderly care setting is an advantage
• Preferably experienced in an acute hospital setting
• Preferably at least 5 years\' experience of the position
