Atlas Care Pte Ltd is a newly set up and fully-owned subsidiary under MOH Holdings Pte Ltd (MOHH).JOB PURPOSEThe Shared Caregiving Services Manager focuses on managing a team of care staff to deliver care services to support the ADL needs and overall quality of life of residents to achieve the typology goals of shortening disability years, active aging, and ageing-in-place.JOB DESCRIPTIONClinical Management

  • Oversee all clinical/care-related matters for the residents
  • Supervise a team of Care staff and roster planning to ensure adequate manpower at all times
  • Assume responsibility in the development, provision, and expansion of care and social services offered by the organisation.
  • Enforce compliance to clinical standards of care and professional code of conduct
  • Oversee competencies and capability building of care staff in the provision of appropriate care to residents, including regular in-house trainings
  • Manage care of residents with complex conditions
  • Case management including escalation to relevant agencies for care and/or social support required
  • Manage regular reporting and tracking, and report metrics
  • Any other duty as assigned by supervisors
Intake Assessment (Upon Admission)
  • Conduct assessment and evaluation of care needs of residents and refer residents to appropriate services
  • Coordinate and implement care plans for residents under their respective service packages
  • Educate next-of-kin of residents on the residents\' conditions and help them gain understanding on the holistic care needed
  • Manage expectations of residents on types and frequency of assistance to be provided under the respective care packages
  • Refer cases to medical social services for financial counselling where necessary
Case Tracking & Documentation (Every 6 months)
  • Maintain proper documentations of all residents\' cases
  • Conduct 6 monthly review of existing residents, and, as and when required, review care needs when condition changes
  • Ensure proper and accurate nursing documentation
  • Liaise and communicate with residents or their next-of-kin regarding issues and recommend solutions
Business Development
  • Organise events and activities to promote the SCS services and attract subscription
  • Regular review of sales packages and prices to ensure relevance to customer needs and business sustainability.
Educational
  • Degree/Diploma in Nursing from a recognised institution by the Singapore Nursing Board
Other requirements
  • Demonstrate good communications and people engagement skills, preferably the use of local languages and dialects
  • Good organisation and leadership ability
  • Able to form networks and strong linkages with stakeholders
Years of Experience
  • At least 3 to 5 years of clinical experience, preferably in public healthcare or eldercare sector.
