Job Summary



Purpose of job:


execution of casework support under the guidance of a social worker, including home visits to engage clients and caregivers living with mental health conditions. Support basic intervention tasks identified by social workers. Engagement of clients with dementia with meaningful activities through befriending and cognitive stimulation programmes. Monitoring of client's conditions and progress. Provide service linkage for client and caregivers. Support the team by providing administrative, operational and logistical assistance for client-related services, programmes, outreach efforts and group work activities.



Key Responsibility Areas



Passion to work with individuals and caregivers with mental health conditions. Basic befriending, , elder sitting duties and emotional support for elderly. Support seniors and caregivers during service duration. Perform basic mental health screening tools for clients and caregivers. Support community outreach, group work and referrals. Ensure proper documentation of case notes, assessment, records.



Requirements



Experience:


Prior experience in working with elderly clients and caregivers in the community setting. Relevant working experience in healthcare or social service sector. Able to co-facilitate group work.
Qualifications:


Professional Cert/Nitec/Diploma


Others:


Requires travel and home visits to various locations within the service area.
