Job Description
Responsibilities:
- Liaise with patients and Medical Social Worker for special cases
- Assess financial and psychosocial needs of cases.
- Provide relevant interventions and referrals for community services.
- Identify high risk cases and refer to Medical Social Worker for further interventions.
- Process applications into National Means Testing System (NMTS).
- Ensure patients are duly accorded with correct means test subsidy
- Process Senior Mobility Fund Applications, Renewal and Terminations.
- Maintain filing
Requirements
- Minimum Diploma in social service
- Experience in handling social work in healthcare industry
- \xef\xbb\xbfPossess good interpersonal and communication skills\xef\xbb\xbf
