- Provision of casework management and counseling through social work assessments and intervention in the management of patients
- Delivery of social work services and programs to patients in need
- Manage patients in the centres through client engagement, conducting interviews, data collection, case assessment, case conceptualization, case planning and intervention plans for best patient outcomes
- Identify presenting and underlying issues and problems to design a care plan to enhance/maintain psychosocial functioning of patients
- Conduct visits to the centres, home or hospitals to provide support counselling
- Collaborate with healthcare professionals in total patient management
- Facilitate patient and caregiver support group programmes
- Min Diploma in Social Work
- Good interpersonal skills
