About The RoleReporting to the Group Supervisor, the Social Work Associate works closely with the team to serve clients andcaregiversKey Responsibilities and Duties. Handle case related enquiry from clients and the community. Make initial contact and coordinate with caregivers and conducting home visit, calls and other engagement. Manage and update clients information and data. Regular monitoring of clients. Provide information and referral services. Assist Social Workers in case assessment, care formulation and planning intervention. Weekly home visitation report. Ensuring submission of application document for FA, e.g. Lee Foundation, claims. Document and maintain case records and prepare quarterly submission including company electronic filesaccording to the required documentation standard. Network, lead and coordinate stakeholders and community partners' engagement. Develop and maintain the database of community partners. Collect and submit data for reporting and evaluation.
