Job Responsibilities

• Assess needs of seniors with care/service needs and formulate care plans for seniors
• Assist in the development and implementation of programmes for seniors through data
• Collection and the conduct of basic analysis
• Coordinate care for the seniors with relevant stakeholders
• Documentation of case notes
• Facilitate timely and appropriate service referral and provision to seniors
• Participate in case discussions with external service providers
• Provide case management services
• Perform needs assessment of seniors living in the community
• Prepare data for submission to relevant authorities

Job Requirements

• Degree in Social Work
• With relevant experience in Social Service Sector or related field is an advantage
• Training provided
