Job Description


:

The Community Case Management Service provides case management service to help vulnerable seniors and their caregivers who face difficulties in managing their multiple health and social care needs to continue staying in the community for as long as possible.

Roles & Responsibilities:

  • Provide intake and triage services for members of the community
  • Conduct psychosocial assessment to plan and coordinate interventions with healthcare and community professionals to support beneficiaries
  • Provide casework, counselling and service coordination for seniors and caregivers
  • Conduct analysis, plan and implement programmes and services to meet clients\xe2\x80\x99 needs
  • Liaise and work with organizations, partners and volunteers to build an ecosystem of assets and resources to facilitate more effective work with seniors and caregivers
  • Assist with report submissions or other projects, as assigned by supervisor
  • Opportunities to be involved in organizational level projects
Job Requirements:
  • At least 2-3 years of relevant working experience with good knowledge in eldercare or healthcare setting
  • Knowledge and experience in eldercare/healthcare sector will be advantageous
  • High proficiency in written and spoken English
  • Bilingual and be able to speak other dialects would be advantageous
  • Strong interpersonal and communication abilities with individuals and systems at multi-levels
  • Good analytical and assessment skills
  • Team player with an ability to function effectively in a fast-paced environment
Qualifications:
  • Bachelor in Social Work or relevant fields (Psychology, Community Work); candidates from non-social work qualification will be considered as Social Work Executive
