Responsibilities

: " About The Role Safe and Strong Families - Reunification (SSF-R) aims to reunify children with their natural families in a safe and timely manner. The objective of this service is as follow: . Reduce the length of stay in Out-of-Home care placement . Reduce recurrence of maltreatment of children . Enable more family reunifications that are timely, successful and enduring .Key Responsibilities of the Role. . Provide intensive, home-based intervention and support to a caseload of 5 to 6 families (high and medium intensity cases), and ensure the children are reintegrated back to their caregivers. . Conduct needs assessment and environment scanning to identify needs of the family using tool such as Structured Decision Making (SDM) and biopsychosocial approach. . Conduct systemic casework and counselling to the family unit, and meet the identified caregiver at least once a week. . Brokerage with various stakeholders from community and governmental agencies in mobilizing social and healthcare resources to meet family's reintegration needs. . Conduct and facilitate case conferences and multi-disciplinary meetings to review the family's progress . Design and implement family-based programs to promote family bonding. . Monitor service implementation and evaluation of service . Maintain a proper and accurate statutory record of the services rendered to family and ensure timely submission to MSF on a monthly basis. Candidate Requirements Technical Competencies: . Bachelor in Social Work or a Grad Dip in Social Work a must . At least 5 years of working experience with high to medium risk families in a social service setting 3. Has experience working with highly dysfunctional families. Core Competencies: . Sound counselling and assessment skills . Empha strength-based intervention and communication . Not biased and judgmental against birth families . Culturally competent . Believe in trauma-informed care and nurturing the family members Preferred achievements / characteristics: - all attributes of SSF-R Social Worker - prior supervisory experience would be a plus - Ability to converse in Malay or Tamil would be an advantage Only short-listed candidates will be contacted for an interview.
