Maintain at least 80% of active cases per quarter of which 100% to be supported with electronic recordings
Conduct assessment, planning and intervention for cases
Participate in internal and external case conferences, networking and case management meetings
Document and evaluate case outcomes
Develop and review appropriate intervention strategies
Adhere to CSWP and SSNet processes in managing cases
Information and Referral
Provide accurate information and refer clients to appropriate agencies for further intervention
Attends to walk-in clients
Adhere to CSWP and SSNet processes in managing referral
Programme - Groupwork / Community Work
To provide support to Coordinator in planning, implementation, evaluation and documentation of the programme
To plan and organise events and activities for clients
Qualifications and Requirements
Degree or Post-Graduate Diploma in Social Work
Min 3-5 years relevant work experience in Social Work setting
Demonstrate ability to handle work professionally, independently and efficiently with a strong customer service mindset
Possess effective organizational and follow-up skills, paying attention to detail
Good interpersonal skills and able to interface with all levels, internally and externally, in a professional and responsive manner.
