Job Responsibilities:
Utilises a holistic, multifaceted, and person-centred approach to meet resident's psychosocial needs
Reaches out proactively to residents and their families to provide a meaningful stay in the Home.
Provide support to residents/family members to assist in their understanding of placement and facility issues in addition to referring them to the appropriate social service agencies when the facility does not provide the needed services.
Regular interactions with residents and their families by making ward rounds daily.
Review facility process and procedures as part of the facility's interdisciplinary team to assure compliance with regulations.
Understand and meet all government requirements for social service documentation.
Triaging of complex cases for pre-admission intakes.
Conduct Bio-Psychosocial-Spiritual (BPSS) assessment, case management and coordination of care for elders and caregivers.
Facilitate and prepare reports for financial assistance applications such as Medifund.
Coordination of LPA, court appointed deputy or Panel deputy, if relevant.
Job Requirements:
Candidate must possess at least Bachelor's Degree/Post-Graduate Diploma/Professional Degree in Social Work or equivalent.
At least 2 Year(s) of working experience in the related field in a Restructured/Community Hospital or Nursing Home
Keen to work in the eldercare sector
