Part of core team in case management and counselling of clients from pris About The Role The Turning Point is a female Halfway House for the rehabilitation of recovering female ex-offenders from substance abuse and related crimes. Part of core team in case management and counselling of clients from prison. Provide case support, risk assessment as well as conduct psychoeducation and rehabilitative programs for the residents Perform direct casework and assistance to client and aftercare planning. Accompany and assist Client in legal, housing, medical, and financial appointments and assessments Monitor medical and mental wellbeing of clients and engage support of their families Monitor and evaluate existing programs and identify to initiate new programs to help in the recovery of clients. Work closely with core team and liaise with external partners, agencies such as Ministry of Social and Family Development (MSF), National of Council and Social Service (NCSS) and other social welfare organizations Ensure timely submission and outcomes of case audit and update of CMS (Case Management System2) Implement Aftercare plan and coordinate Befriender and support groups for client reintegration needs. Reports to Case Manager. Degree in Social Work or Sciences or Diploma or experience in social work 2-5 years of relevant experience 5 working days including one weekend day Work Location at Sembawang Candidate Requirements Technical Competencies: Ability to engage in case management in consultation with core team and with community partners in rehabilitation needs of client group. Preferably registered with SASW and or SAC Ability to analyze, interpret policies and schemes that impact client group. Core Competencies: Passion for service, resilience, commitment and team player Preferred achievements / characteristics: Program and project management and co-ordination experience Ability to drive is an added advantage.
