Social Worker | Senior Care Centre

Job Description
:
- Provide comprehensive case management and supportive counseling for seniors and their family caregivers.
- Conduct psycho-social and financial assessments to inform personalized care plans and clinical interventions.
- Plan, develop, and implement community programs and support groups tailored to the needs of seniors and caregivers.
- Facilitate therapeutic support groups and conduct home and hospital visits as part of ongoing client support.
- Serve as a key liaison, building and maintaining strong networks with relevant agencies and stakeholders to advocate for client welfare.
- Min Degree in Social Work
- At least 3 years in Social Service Sector
- Good interpersonal and problem-solving skills
- Proficient in English, bilingual candidates with fluency in local dialects as role involve to communicate with dialect speaking clients (ability to translate dialect to English)
- Working Hours: Weekdays 9am to 6pm
