Conduct assessments for intake triage for ABLE programs
Ensure comprehensive documentation where all relevant information related to client care is recorded and available to all members of the multi-disciplinary team
Follow up on case management work and administrative work regularly
Formulate and execute relevant psychosocial interventions to improve the well-being of clients and caregivers in consultation with multi-disciplinary team.
Counsel and conduct financial assessments of clients and their families, and recommend appropriate interventions to support holistic client and caregiver management
May perform field work involving home and work site visits
Implementing, lead initiatives and support programmes that enhances the care and support of client and their caregivers within the community
Lead in multi-disciplinary team meetings and facilitate family conferences and case discussion where necessary
Experience in runing group work (e.g. client/cargiver support group)
Any other duties assigned
Return-to-Work Coordination Coordinate and support clients on ABLE\xe2\x80\x99s Return-to-Work programme by reintegrating them into suitable and sustainable employment:
Provide vocational support to guide clients in goal setting and actualization
Liaise between healthcare providers, client and potential employers to facilitate clients\xe2\x80\x99 early return to work, including addressing the need for light duties or accommodations where relevant
Provide pre- and post-placement support to enable clients to settle in and sustain employment
