Job Description


:The KidSTART Practitioner\'s role is to provide targeted assistance to children and families enrolled in the KidSTART programme. Key responsibilties include:A. Intake and Referral

  • Process referrals from a variety of sources, including hospitals, community referrals, self-referrals, government agencies and other Social Service Agencies (SSAs)
  • Engage families about the programme, seek consent from and determine eligibility of families for KidSTART programme.
  • Collate and maintain accurate and complete set of data in the Social Service Net (SSNet) and on internal databases for all referrals and intakes.
B. Needs Identification and Assessment
  • Conduct regular screening and assessment using different tools and assessments to ensure child\'s developmental needs are met and support maternal/main carer\'s well-being.
  • Develop holistic child-centric case plans to coordinate required support for the identified children.
C. Case Coordination, Systems Linkages and Mobilisation
  • Work closely with community partners/stakeholders to enhance service delivery and promote positive child development outcomes.
  • Where needed, organise case conferences involving relevant parties to coordinate support and address barriers in support for identified children and families.
  • Tap on a team of multi-disciplinary professionals to support health and developmental needs of identified children.
  • Coordinate support for identified children and their families through relevant KidSTART support or other resources.
D. Education & Equipping
  • Conduct regular parent engagement sessions to enhance caregivers\' knowledge, skills and parenting capacity based on the KidSTART curriculum.
  • Upkeep and maintain all required administrative documentation including case notes.
Requirements:
  • Require to attend to clients who are Mandarin, Malay or English-speaking. High proficiency in Mandarin or Malay will be essential besides proficiency in written and spoken English.
  • Organized and resourceful
  • Strong interpersonal and communication abilities with individuals and systems at multi-levels
  • Enjoy working in a dynamic social service start up environment
  • Enjoy working on a range of tasks and responsibilities related to direct work and service planning
  • Team player with an ability to function effectively in a fast-paced environment
  • Desire to participate in shaping an early childhood social work intervention model
