Social Worker/social Work Executive

Job Description
:
- Perform community profiling and identify the community needs and assets within the central/north region of Singapore
- Engage with relevant social and healthcare stakeholders to develop and pilot workflows, protocols and processes to facilitate social-health integration
- Set up linkages and protocols between different community providers to manage clients across multiple social-health services
- Review and track the effectiveness of inter-agency workflow in facilitating care transition and provision
- Develop and implement new initiatives to meet the social and health needs of the community
- Provide information and referral services
- Provide monitoring and coordinate care services for seniors
- Bachelor\xe2\x80\x99s in Social Work or relevant fields (Psychology, Community Work); candidates with non Social Work degree will be considered as Social Work Executive
- At least 2-3 years of relevant working experience with good knowledge in eldercare or healthcare setting would be advantageous
- Strong interpersonal and communication abilities with individuals and systems at multi-levels
- Good facilitation skills
- Team player with an ability to function effectively in a fast-paced environment
- Possess Bachelor\xe2\x80\x99s in Social Work or relevant fields (Psychology, Community Work)
