Job Description

We are looking for an enthusiastic and self-motivated pharmacist to join our dynamic team in providing a comprehensive service to our oncology/haemato-oncology patients. The interested candidate will be given supervised in-house competency training and accreditation in the outpatient setting.The key responsibilities are: . Verification and screening of prescriptions of systemic anticancer therapies (e.g. chemotherapy, immunotherapy). Medicines reconciliation and clinical counselling to patients. Provision of drug information to patients, nursing, pharmacy and other healthcare professionals. Provision of advice and support to doctors whilst ensuring the safe, appropriate and cost-effective use of medicines. Educating colleagues within NHG and supporting their training. We have trained and supported KTPH pharmacist, and have been heavily involved in the start-up of their oncology services.. Development of protocol and guidelines for treatment, together with nursing, medical and other healthcare professionals. To advise on the safe management of high alert medicines. Promoting a patient focused multidisciplinary approach to the delivery of an efficient, high quality pharmacy service.There will be opportunities to: . Be trained and perform medication review of OP oncology/haemato-oncology patients. Rotate to clinical trials and inpatient for training. Pursue oncology specialisationPortfolios of current team members include: . Subject matter experts in oncology/haemato-oncology/clinical trials contents/workflow for NGEMR (BEACON). Risk assessment and management in various areas, e.g. Medication safety team. Working closely with Medical Oncology/Haematology/Dietetics department to streamline protocol and produce guidelines for patient treatment. National PIL leads for oncology/haemato-oncology drugs. Supporting patient education in oncology/haemato-oncology drugs e.g. counselling videos and materials. Horizon scanning of new drug development, indications and producing regular informational bulletins for medical, nursing and pharmacy colleagues. Setting up and streamlining drug programmes to aid cancer patients with healthcare costs. Setting up clinical trials in line with government regulations. Providing expertise as a member of the Drugs & Therapeutics committee. Providing support and input as a member of Joint Oncology Innovation Taskforce (JOINT) - e.g. contribute to the NHG Oncology Strategy Plan re: training, services and resources for KTPH and WHC.. Providing input into the new build in the ambulatory block as a member of the Oncology Line Sub-CommitteeRequirements- Full registration with Singapore Pharmacy Council- Bachelor Degree / Masters in Pharmacy Bachelor in Pharmacy with minimum 2 years of experience- Able to work in a fast-paced environment
