What the role is:The functions of the Social Support Group are to: a) Develop operations policies for and deliver social assistance schemes and programmes, and address system barriers to support lower-income and vulnerable individuals and families; b) Develop, manage and license MSF-funded social services for lower-income and vulnerable individuals and families; & c) Develop partnerships with the community and corporates to deliver services and support to lower-income and vulnerable groups. Description of Division/Branch: The Residential Support Branch under Residential Support & Licensing Division provides support to the vulnerable individuals in the community, including rough sleepers, homeless persons, cardboard collectors, tissues sellers, and those requiring subsistence or accommodation support.What you will be working on:The Central Placement and Case Support team of the branch focus on collaborating with stakeholders to support the various types of vulnerable persons for placements into appropriate facilities based on their needs and to ensure that these individuals reviewed regularly to assist in their rehabilitation and reintegration back into the community. Responsibilities: \xe2\x80\xa2 Referral Processes - To streamline, review and refresh to keep referral process relevant and to reduce waiting time for rough sleepers and destitute persons awaiting placement. Work with Data Team to Design and Develop systems interface to provide more \xe2\x80\x9clive\xe2\x80\x9d data on vacancies for smoother and faster referral placement. \xe2\x80\xa2 Efficient matching of rough sleepers and destitute person to accommodation - Work with and engagement of partners and stakeholders in supporting rough sleepers and vulnerable individuals such as facilitating placement of the individuals into shelters and welfare homes. Monitor the capacity and vacancies across various shelters and welfare homes. \xe2\x80\xa2 Social Investigation - Investigate into the social circumstances of the Homeless / Destitute Person and Assist the Homeless / Destitute Persons in appropriate accommodation placement. Works closely with other agencies such as SSOs, FSCs, Singapore Police Force, Immigration and Checkpoints Authority, National Environment Agency, National Parks Board, Housing and Development Board, etc. Monitors that the homeless / destitute persons are right sited in accommodation placement with follow-up plans. \xe2\x80\xa2 Oversight of Case Management of Residents in shelters and welfare homes - Overview of the rehabilitation and reintegration of these individuals back into society by case management oversight of the assistance rendered to assist them address and sort out their social, health, financial, employment, accommodation and other issues. Review cases to ensure that residents are care for and residential rehabilitation continue to be relevant and required. Centrally monitoring and managing the repository of data collected on residential care needs. \xe2\x80\xa2 Secretariate and Staffing Work - Perform secretariate work for the Review Committee and other committees. Plan, schedule, and coordinate the meetings with the various stakeholders as well as follow-up with the shelters and homes on the cases in their residential care.What we are looking for:
