Since October 2011, Thye Hua Kwan Moral Charities (THKMC) was incorporated as a charity with an IPC status to provide multiple social and welfare services to the community at large. THKMC provides assistance to the various groups in our community through more than 60 programmes and services for the elderly and the sick, families, persons with disabilities, and children. We help anyone in need with full respect to their race, colour, creed, language, culture and religion. DESIGNATION : Staff / Enrolled Nurse
RESPONSIBILITIES
DUTIES & RESPONSIBILITIES
- Conduct screening of new referrals.
- Conduct assessment of clients in their home environment and develop an individual care plan for clients.
- Perform basic nursing procedures e.g change of NG tube, wound dressing, catheterization, medication packing etc.
- Work with a multidisciplinary team and co-manage clients with other service providers, if required.
- Provide appropriate health education.
- Provide caregiver training and support the caregiver in navigating the eldercare services, including their own psychosocial needs.
- Maintain timely and accurate documentation of client\xe2\x80\x99s condition and care assessment.
- Identify and report risks and barriers to the safety of clients in accordance with organisational protocols
- Execute responsibilities as per emergency protocols in the event of public health threat or emergency
- Comply with guidelines and policies to ensure client confidentiality and personal data protection
- Assists with medical consumables stock-taking and procurement
- Care of medical equipment
- Implement quality improvement initiatives, evidence-based practice and participates in research projects
- Perform other duties as assigned
QUALIFICATIONS
Requirements
- Diploma in Nursing with 4 years of general ward nursing experience. Candidates with Specialist Diploma in Gerontology and/or Advanced Diploma in any discipline is an advantage.
- Registered with the Singapore Nursing Board
OTHER INFORMATION
Working hours: Monday to Friday. 8.30am to 6pm.
