Staff Nurse - Home Care Services

Work Location:

Simei Street 1, Singapore

1?? Staff Nurse - Palliative Care (x3)

Working Hours:

Monday to Friday, 8:30 AM - 5:30 PM

Salary:

$3,000 - $4,500/month

? Allowances:

On-call allowance + Fixed monthly community care nursing allowance

Key Responsibilities:

Conduct home visits for referred palliative care patients Perform nursing assessments and implement care plans Adjust medications under doctors' instructions for symptom control Educate and support caregivers on nursing procedures and emotional care Collaborate with medical professionals for patient care Refer patients/families for financial aid or counselling when needed Provide bereavement support and ensure holistic patient care Achieve at least 6 visits per day

Requirements:

Diploma/Degree in Nursing or equivalent Registered with the Singapore Nursing Board Minimum 3 years of relevant experience Nursing Certificate in Palliative Care

2?? Staff Nurse - Chronic Illness Home Care (x2)

Working Hours:

Monday to Friday, 8:30 AM - 5:30 PM

Salary:

$3,000 - $4,500/month

? Allowances:

On-call allowance + Fixed monthly community care nursing allowance

Key Responsibilities:

Conduct home visits for patients with chronic illnesses Perform nursing assessments and coordinate follow-ups with doctors Carry out nursing procedures (e.g., wound dressing, catheter insertion, NGT care) Educate caregivers/families for continued home-based care Maintain accurate documentation and patient confidentiality Achieve at least 7 visits per day Participate in fundraising activities when required

Requirements:

Diploma/Degree in Nursing or equivalent Registered with the Singapore Nursing Board 1-2 years of relevant nursing experience preferred
