Conduct assessment of clients in their home environment and develop an individual care plan for clients.
Perform basic nursing procedures e.g., change of NG tube, wound dressing, catheterization, medication packing etc.
Work with a multidisciplinary team and co-manage clients with other service providers, if required.
Provide appropriate health education.
Provide caregiver training and support the caregivers in navigating the eldercare services, including their own psychosocial needs.
Maintain timely and accurate documentation of client's condition and care assessment.
Identify and report risks and barriers to the safety of clients in accordance with organizational protocols.
Execute responsibilities as per emergency protocols in the event of public health threat or emergency.
Comply with guidelines and policies to ensure client confidentiality and personal data protection.Additional duties:
Assists with medical consumables stock-taking and procurement.
Care of medical equipment
Implement quality improvement initiatives, evidence-based practice and participate in research projects.
Perform other duties as assigned.
Qualifications: Diploma in Nursing with 4 years of general ward nursing experience. Candidates with Specialist Diploma in Gerontology and/or Advanced Diploma in any discipline is an advantage Registered with the Singapore Nursing Board Valid BCLS and AED certification Able to perform basic nursing procedures independently. The ability to speak dialects is an advantage. Other Information:
