Job Description

Job Responsibilities:

Provide personalised, and cost-effective care to the residents in the nursing home in accordance to established policies and procedures, standards of nursing care, and practices. Provide a holistic, multifaceted and person-centred approach to plan and execute care to meet resident's nursing needs. Able to harness team's cooperation and inspire them to deliver effective, safe, and coordinated care to all. Reaches out proactively to residents and their families regarding discharge. Plans, trains, and coordinates a smooth transaction from a nursing home to a home for them. Report to Nurse Manager/Head of Nursing

Job Requirements:

Candidate must possess at least Bachelor's Degree/Diploma in Nursing Possess a State Registered Nurse license Valid Practising License with Singapore Nursing Board At least 2 Year(s) of working experience in an acute / community hospital or nursing home Possess interpersonal skills and is passionate working with elderly.
