Job Description

:

• Provide nurse counselling and patient education on chronic disease management and health promotion.
• Able to assess patient\xe2\x80\x99s nutritional needs and provide basic nutrition education.
• To teach specific tests for monitoring of chronic diseases e.g. blood glucose, blood pressure etc.
• Tracking of patient\xe2\x80\x99s progress in chronic disease management and lifestyle changes. This can involve face to face consult, teleconsult and telehealth monitoring.
• Case management of complex chronic cases which may involve home visits to assess patient\xe2\x80\x99s home environment, family/caregiver support, mobility and social status.
• Conduct of Diabetic Foot Screening and Foot Care education. Referrals of \xe2\x80\x9cAt Risk\xe2\x80\x9d cases to the Podiatrist based on PCN\xe2\x80\x99s protocol and in consultation with the doctor.
• Work closely with the Primary Care Coordinators (PCCs) in care coordination and care transition. This will involve liaising with the public institutions, GP clinic team, community health and social providers.
• Participate in PCN meetings, CME activities and case discussions.
• When needed, function as a member of the frontline clinical workforce and support the doctors in various clinical activities
• Provide treatment room duties - point of care testing, DRP and DFS
• Provision of basic nursing duties \xe2\x80\x93 ie phlebotomy, injections and wounds dressing
• Providing assistance to ensure smooth and efficient during clinic operations
• Perform triage and assist during emergencies
• Any other duties as delegated by the Medical Director or Director Healthcare Services.

• Diploma/Degree in Nursing or equivalent with at least 3 years of clinical nursing experience.
• Registered with Singapore Nursing Board
• Self motivated and able to work well in a team.
• Good interpersonal and communication skills.
• Proficient with Microsoft Office Applications.
• Able to work in a dynamic environment as well as being flexible and adaptable to changes.
