Participates in the planning, delivery and evaluation of patient care within the haemodialysis
clinical setting: \xe2\x88\x92 Undertakes clinical/patient assessments pre, during and post dialysis treatment to evaluate effectiveness of care \xe2\x88\x92 Adheres to all FMC Policies and Work Instructions related to clinical and operation management \xe2\x88\x92 Reviews blood test results and report to Medical Director of any abnormalities \xe2\x88\x92 Educates patients on their diet, fluid, medications and blood pressure management to optimise their clinical outcomes \xe2\x88\x92 Motivates and encourages patient\xe2\x80\x99s adjustment to CHD \xe2\x88\x92 Monitors the water treatment system and fluid for dialysis to ensure provision of safe, quality dialysis therapies
Establish an effective relationship with patients and maintain good communication with
patients and relatives; and give adequate support.
Ensures/promotes a safe, secure and healthy working environment to staff and a
comfortable, safe environment for visitors and patients as detailed in the IPC and environment policies
Ensure all equipment is clean, safe and in good working condition; and report defects
through appropriate channels.
Maintains accurate patient information in medical records, including Therapy Data
Management System (TDMS)/EuClid Clinical Information system and abides by confidentiality and Privacy Policy.
Protects the rights of patients by adhering to Privacy Principles and providing an excellent
standard of care
Actively participates in the quality improvement process by reporting all incidents, hazards
