Job Description

Position: Temporary Case Tracker
Department: Case Management Unit
:
Case Tracker plays a crucial role in coordinating patient appointments and support patient care services. Case trackers' roles are to ensure patients turn up for their treatment and appointments by coordinating and following up timely and closely.
Job Responsibilities:

Identify patients who have defaulted on appointments and develop suitable action plans.
Conduct telephonic triaging to assess and identify patient needs.
Schedule patient appointments accurately within the system.
Close service gaps by ensuring prompt follow-up on missed appointments and fostering patient engagement.
Provide crisis advice when necessary, during patient interactions.
Handle all calls professionally and courteously.
Manage enquiries and handle difficult calls related to patient appointments.
Communicate clearly and effectively with both internal teams and external customers.
Utilize various IT systems efficiently to perform daily tasks.
Maintain accurate records of patient interactions and appointment statuses.

Requirements:
Competency
Service-oriented, confident and have a passion for helping others
Good communication and interpersonal skills
Contact Center / Healthcare experience will be an advantage.
Educational and Professional Requirements
At least GCE 'O' Level or equivalent with customer service experience.
Proficient in Microsoft Office applications.
