Working Hours: 5 days Office Hours

Job Scope:
Manage, coordinator and facilitate project independently with different stakeholders
Manage cases in accordance with established practices in care delivery and coordination of multidisciplinary practices
Monitor, track and record variance from standard care protocol
Support in the care management and arrangement
Data collection, data entry, and perform outcome measurements, report generation and presentation preparation.

Requirements:
GCE 'A' Level / Diploma in relevant healthcare disciplines (e.g. nursing / pharmacy/ psychology / social work / health sciences)
Possess good interpersonal skills
Basic IT proficiency skills
Able to handle and maintain complex datasets, and multitasked
