Facilitate and conduct Advance Care Planning discussion with patients and next of kin at the inpatient, outpatient and community settings
Conduct talks or workshops to raise awareness and advocate for Advance Care Planning
Responsibilities and Duties:Specific
Process and manage referrals for ACP discussions from inpatient, outpatient and community settings of the organisation
Facilitate ACP discussions with patients, families and nominated spokespersons
Document ACP discussions at the appropriate platforms
Coordinate between patients, families and healthcare team for ACP facilitation and follow up
Make referrals to community services related to ACP matters
General
Work with ACP Coordinator to provide support in ACP referrals and enrolment and data input as needed
Conduct talks and workshops for healthcare professionals to raise awareness about and advocate for Advance Care Planning
Participate in public awareness events (within the organisation or jointly with partners) to raise awareness about Advance Care Planning
Participate and assist in conducting audits, research, surveys or quality improvement projects related to ACP
Preparation of reports and statistics as required.
Job Requirements:
Professional qualification in Nursing, Social Work, Allied Health or/and any clinical disciplines relevant to the job.
At least 3 years of experience as an Allied Health Professional
Experience in a Hospital setting preferred
Certification in Advance Care Planning Facilitation (General and Preferred Plan of care) and experience in conducting ACP discussions would be an asset
