Care Coordinator (advance Care Plan) –

Job Description
- Responsible to facilitate and drive Advance Care Planning for targeted group of patients
- Engage all suitable referrals in Advance Care Planning discussion
- Ensure effective communication of patients\' Advance Care Plans to different stakeholders (Eg: Allied health professionals, patients\' family members etc)
- Follow up and review outcome of Advance Care Planning
- Follow up on concerns raised during Advance Care Planning discussion
- Assist to raise awareness of Advance Care Planning and conduct Advance Care Planning advocacy and facilitator training
- Degree in Nursing, Social Work, Psychology, Allied Health or equivalent
- Good interpersonal and communication skills
- Proficient in using MS office
- Knowledge in statistics and familiarity with relevant community services will be an advantage.
