Job Description


:

Join the Community Resource Engagement and Support Team (CREST) to support persons newly diagnosed with dementia and provide their caregivers with a structured support system consisting of information, service linkage, and case management. As a Care/ Assistant Care Manager, you can look forward to working closely with internal and external stakeholders such as regional health services, hospitals, volunteers, grassroots, schools, and more, to plan activities and programs for seniors.

  • Programme Development and Implementation
  • Plan, implement, and monitor the care process, referral, and follow-up workflow.
  • Provide care coordination and service linkage to clients with dementia and their caregivers, with the appropriate resources and support including caregiver support.
  • Be responsible for establishing a network with other stakeholders like community partners and/or related agencies to build collaborative partnerships and better facilitate care and support for clients and caregivers.
  • Establish and review work and communications workflow including identification, referral, and escalation work processes as well as the development/update of the CREST-PDS service manual.
  • Administration and Staff Development
  • Review and analyze collated data and monitor the team\xe2\x80\x99s progress and performance.
  • Ensure that claim and data submission to AIC is timely and accurate.
  • Ensure programme data and documents are kept and maintained, in accordance with the CREST programme requirement, as well as the prevailing personal data protection and security guidelines.
  • Participate in relevant meetings with key stakeholders including AIC to share key learning points where required.
Qualifications:

For Care Manager: Bachelor\xe2\x80\x99s Degree preferably in Social Work / Psychology / Sociology / Gerontology / Counselling

For Assistant Care Manager: Diploma with at least 3 years of social service experience/ Diploma in Social Service/Gerontology
  • Proficient in Google Suite Applications
  • Excellent service mindset to serve the community
  • Possess good interpersonal and communication skills
  • Pleasant, proactive, and highly accountable for the work assigned
  • Able to work independently and meet deadlines
  • Able to work independently and at the same time as a team player
Additional Advantageous Skills:
  • Ability to converse in local languages
