Singapore
Plan, develop and review the services and operations of the Specialized Case Management Programme (SCMP) and Take-A-Break or TAB (caregiver respite). He / she also reviews the post-18 service continuum, and co-lead or develop initiatives within the adult service landscape or for SPD, to meet emerging needs of the clients and caregivers, or for SPD strategic development.

Responsibilities

Administration

o Responsible for all matters pertaining to administration, operations and control of the SCMP/TAB programmes.

o Recommend policies and practices for the efficient and effective operation and management of services under your purview as well as implementing those policies and practices.

Operations

o Drive outreach activities to enroll persons with disabilities (PwDs) and caregivers.

o Develop and implement a slew of SCMP/TAB programmes to engage PwDs and caregivers.

o Supervise a team including social workers and administrative staff, and work with various government as well as community-based organisations to offer services to adults with disabilities.

o Lead the team in developing the service model / review, setting service guidelines/standards and monitoring service delivery of programmes.

o Responsible for achieving key performance indicators, providing progress reports and assisting in data analysis for planning and reporting purposes.

o Keep abreast of latest development in service delivery of related areas to ensure that service delivery for our clients are updated.

o Submit indicators to all relevant funding agencies such as MSF and SG Enable etc. Coordinate the claims for submission to funding agencies.

o Prepare annual budget for the centre, and relevant papers where required.

Service Development & Sustainability

o Engage in organizational planning i.e. strategic planning, programme planning and services review of the programmes

o Ensure the sustainability of the programmes, including funding exploration and renewals.

o Keep current with the funding and staffing model for the programme.

o Prepare annual budget for the programme (SCMP/TAB)

Networking

o Responsible for outreach to referral agencies, community organizations and hospitals for client referrals to SCMP/TAB

o Participate in events organized or supported by SPD and develop strong networks with other Social Service Agencies (SSAs), community organizations and funding bodies like MSF, NCSS and SG Enable, towards active collaborations.

Requirements

Master's or bachelor's degree in any discipline related to social work, or health science from a good University. At least 5 years of relevant working experience in managing programmes for persons with disabilities. Excellent management and leadership skills with demonstrated ability to lead a team. Communicative, persuasive and able to encourage and motivate the team. * A good understanding and knowledge of the sector and dynamics.
