Job Description


:

  • Assist the Head of Department (HOD) in providing supervision guidance and precepting assigned staff to ensure works are appropriate quality and standard
  • Assist HOD in assessing staff competency and identify learning needs
  • Assist HOD in reviewing and update policies and procedures to meet ministry regulatory requirement
  • Assist HOD in reviewing and develop workflows to ensure seamless admission of residents
  • Collaborate with government agencies, hospitals, community service providers, internal and external healthcare teams to provide coordination and continuity of patient care across healthcare continuum
  • Conduct and participate in case conference
  • Act as a coordinator for all subsidized admissions under (MOH) for ENH/EMC/SVH
  • Perform financial counselling for all residents including the preparation of admission contract and other administrative matters
  • To perform FA/HHMT when needed
  • Assist the resident and their families for any government subsidy application i.e. conversion / transfer internal and external
  • Perform assessment on the resident and their families\' eligibility for financial assistance
  • Assist HOD in putting up the following application for the resident and families:
  • Day Care
  • Day Rehab
  • Home Modification
  • Senior Mobility Fund etc
13. Assist HOD in the applications for government financial schemes for the resident and families. Applications would include the following, and other schemes that may be available
  • Interim Disability Assistance Programme for the Elderly (IDAPE)
  • Eldershield Claim
  • Medical Fee Exemption Card (MFEC)
  • Medifund Claim
  • PGDAS etc.
14. Perform such other duties and responsibilities that may be assigned from time to time15. Put up Medifund claims for all EMC/ENH/SVH subsidized residents16. Ensure all Medifund claims are submitted duly17. Ensure all MFEC validity and application18. Ensure all supported documents are updated for all claimsRequirements:
  • Degree in Social work or any equivalent qualification
  • At least 3 years relevant working experience
  • Ability to work independently
  • Ability to multi-task
  • Ability to work effectively in team environment
  • Possess effective communication and interpersonal skills
  • Empathetic and compassionate
  • Proficient with MS Office
  • Ability to work under pressure and meet tight deadline
