Develop clinical programmes/services mapping framework across health hand social care setting and internally
Develop clinical programmes/services mapping toolkit to enable a consistent and systematic process to map the programmes/services with partners across health and social care settings and internally
Develop mapping of current landscape / patient care journeys for identified population profiles together with providers across health and social care settings to gain insights on current care gaps
Work with Corporate Communications to develop relevant engagement material that is appropriate for the partners to be engaged to articulate the care models developed
B) Care Re-design
Co-drive care redesign workshops with internal stakeholders and partners in community develop new models of care that will transform care delivery at a population health level
Develop new models of care that will enable care integration at a population level
Develop integrated care programmes / services as part of ICO project(s) that will be delivered by the network of care providers
Develop operational model and implement the programme(s) as part of the demonstration project(s).
Project management of the programme(s) including developing the programme(s) dashboard.
C) Regional Health Office secretariat
Be part of Regional Health Office secretariat to drive agenda for Woodlands Health Integrated Care Organisation (ICO).
Organise meetings and drive discussions with Woodlands Health ICO Ex-Co.
Liaise and coordinate with NHG ICE for discussions with NHG ICE.
General
Map current programmes and services internally and externally
Develop patient care journeys with internal and external stakeholders across all settings in the population.
Develop new models of care that will enable care integration at a population level.
Develop int egrated care programmes/servi ces as part of ICO project(s) that will be delivered by the network of care providers.
Develop operational model and implement the programme(s) as part of the demonstration project(s).

Job Requirements
University Degree in any discipline from a recognised institution, preferably in health and social sciences or business degrees
At least 5 years of related work experience in healthcare, community or primary care sector in Assistant Manager role
At least 8 years of related work experience in healthcare, community or primary care sector in Manager role
Preferably with experience in account management and/or customer relations
Project management skills
Strong interpersonal, community and stakeholder engagement skills
Able to facilitate discussions and translate concepts into frameworks and care and operational models
Good analytical and writing skills
Passion in transforming public healthcare
Self-motivated, driven and initiated.
