Job Description

Responsibilities:
- Support the care team in coordinating and improving the patient\xe2\x80\x99s care
- Work closely with other nursing and allied team to provide advocate care options and services to the patients and their family members
- Improve the patient\xe2\x80\x99s care plan by understanding their needs
- Involve in community outreach programmes
- Degree in Health related fields
- Working experience in healthcare or social service sector is an advantage
- Possess care management experience
- Positive thinker and good team player
- Passion for service excellence and process improvements
- Ability to multi-task and work in a fast-paced and dynamic environment
