• Respond to medical queries through a 24/7 hotline that connects patients, caregivers, and community partners for health information within the community. Ensure each query is directed to the appropriate medical provider and follow up to ensure proper resolution. Review patients\' conditions two weeks after their initial call. Handle non-medical calls effectively.
• Provide guidance and support to a team of Care Manager Associates, oversee their schedules, and manage shift duty claims. Work rotating morning and afternoon shifts, and be on standby for night shifts if needed. Support the expansion of the Virtual Care Centre (VCC) and attend meetings with both internal and external stakeholders.
• Participate in activities to improve patient care, including professional development sessions. Engage in projects and community events or its partners. Perform any other duties as assigned by the Reporting Officer.
