Our Client, a leading and well-known Health Institution in Singapore is inviting qualified candidates to fill the position as Care Coordinator.Candidates with relevant healthcare experiences are welcome to apply!Summary:

Responsibilities:

• Assist in the care management team to provide basic screening and measurements for chronic patients.
• Educate patient on home monitoring devices
• Assist in planning, organizing and coordinating workplace health promotion activities for staff and perform any other duties as assigned by supervisors.
• Make appropriate referrals to care managers, assist patients to schedule follow-up appointments for consults, screenings and vaccinations in relation to chronic care management/population health.

Requirements:

• A team player with good interpersonal & communication skills
• Passionate to work in Healthcare industry
• Customer-oriented, a team player, a fast learner and serious with work
• Relevant experience in healthcare industry
