Our Client, a leading and well-known Health Institution in Singapore is inviting qualified candidates to fill the position as Care Coordinator.

- Assist in the care management team to provide basic screening and measurements for chronic patients.
- Educate patient on home monitoring devices and use of Healthhub portal.
- Assist in planning, organizing and coordinating workplace health promotion activities for staff and perform any other duties as assigned by supervisors.
