Job Description


:The case management officer is primarily responsible for the delivery of neuropsychological assessments and the arrangement of care plans for psychiatric patients, through liaising with various stakeholders, such as family members, doctors, allied health staff and external agencies S/he is expected to facilitate the smooth running of psychiatric clinics by performing a range of clinical and administrative duties.Primary Responsibilities and Duties

  • Administer neuropsychological assessments and collect socio-demographic information of new outpatient patients and follow-up with patients as required.
  • Obtain clinical information including medical, psychiatric, social, and family histories from patients prior to patient\'s first visit in the clinic.
  • Contact patients prior to their first visits to confirm their attendances, remind them of their first visit and follow up appointments, and brief them on the administrative steps for the online application platform (if any).
  • Triage cases based on their clinical needs under the supervision of a Psychiatrist.
  • Provide psycho-education for patients and their families/ caregivers.
  • Develop comprehensive treatment plans for assigned patients based on assessment findings, diagnosis, and identified needs. Coordinate and monitor implementation of treatment plans, including monitor patient\'s appointments, investigations, social and rehabilitation plans.
  • Provide ongoing case management services through scheduled follow-ups and check-ins. Assess progress, identify any emerging needs, and adjust treatment plans accordingly.
  • Collaborate with multidisciplinary team including Psychiatrists, Allied Health staff, Nurses and other professionals for the management of patient\'s assessed needs.
  • Familiarize with community resources and connect patients and/or caregivers with services to AIC, day care centers, family care centers etc. as necessary.
  • Facilitate discharge to step-down care (Polyclinics, GP, St Luke Hospital etc.) and ensure effective and smooth transitions/ follow-up care.
  • Maintain timely and thorough progress notes and updates within patients\' medical records.
  • Communicate with patients\' families/ caregivers to provide status updates, discuss treatment progress, and enroll them in supporting recovery.
  • Assess caregivers\' needs with appropriate assessment tools and arrange appropriate referrals for support.
  • Ensure that relevant patient education materials are available and are up to date.
Secondary Responsibilities and Duties
  • Perform data management, collate and maintain patient database for clinical and research purpose.
  • Assist in tracking, maintaining and reporting of quality clinical indicators and provide statistical report and updates on a regular basis.
  • Participate in projects related to case management and clinical pathway.
  • Perform other duties and responsibilities, as necessary, in the overall operation of the clinic.
Job Requirements:Qualification:
  • Degree in Nursing, Psychology or relevant discipline.
Experience:
  • Relevant experience will be advantageous.
  • Good knowledge of community resources and experience with care coordination.
  • Computer literacy skills - MS Office (Word / Excel / PowerPoint).
  • Strong organisational, coordination, interpersonal and communication skills.
  • Ability to collaborate effectively in multidisciplinary teams and to work independently.
