Join our dynamic and vibrant department as a Case Management Officer (CMO) in Yishun Health Campus and make an impact on the lives of patients and their families. We are seeking compassionate and trained individuals to support our acute, community hospital and outpatient clinics.As a CMO, you will be responsible for providing psychosocial care to our patients. Working closely with multi-disciplinary team to establish a holistic care plan and carry out appropriate interventions are essential aspects of the work as a CMO.Our campus adopts Kaizen approach and looks for continuous improvement when caring for patients. You will be given opportunity to support in improvement projects/ innovation and learning activities in enhancing the effectiveness and efficiency of our hospital services.Qualifications:

  • Must have attained professional qualifications either in nursing, medicine, or para-clinical disciplines
  • Have prior job experience in either nursing, medicine, or para-clinical discipline with at least 5 years working experience in patient related care
  • A team player who enjoys working in a fast-paced environment
