Job Description

:
The Case Manager (Social Worker) will be part of the COMIT Team who will ensure service delivery is in accordance to the intended programme objectives and care model for the programme as outlined under the terms & conditions of the Letter of Award/Addendum.
The Case Manager will work closely with the Case Integration Lead to oversee the triaging process, reviews and work towards achieving the COMIT goals of timely assessment, provision of intervention and support eg. psychoeducation, case management and counselling, as well as caregiver support and training.
In addition, the Case Manager (Social Worker) may also be involved in networking with other community partners or agencies, for purpose of collaboration and to facilitate co-management of cases where applicable and other adhoc duties assigned.
This candidate will be one of the key members driving the COMIT project.

  • Assist with the triaging process for the AIC COMIT Programme.
  • Ensure programme data and documents are kept and maintained, in accordance to the COMIT programme requirement, as well as the prevailing personal data protection and security guidelines
  • Work closely with the Case Integration Lead to oversee the triaging process, reviews and work towards achieving the COMIT goals of timely assessment, provision of intervention and support eg. psychoeducation, case management and counselling, as well as caregiver support and training.
  • Participate in relevant meetings with key stakeholders including AIC to share key learning points where required
  • May also be involved in networking with other community partners or agencies, for purpose of collaboration and to facilitate co-management of cases where applicable.
  • Any other adhoc duties assigned.
Requirements:
  • Recognized Bachelor's degree in Social work.
  • Has at least 3 years of working experience with case management, preferably in the Healthcare / Social service sector.
  • Team player with good interpersonal, oral and written communication skills.
  • Preferably with supervisory experience, able to provide case supervision to the team to provide intervention and support through the project.
  • Work experience in social service sectors or familar with the ASCAT/COMIT Programme would be an advantage.
