Job Summary:

• Working location: Central
• Working days & hrs: 5 days; Monday to Friday ;9.00am - 6.00pm
• Salary range: up to SGD 4,000 (Dep on Exp.) + AWS

Primary Responsibilities

• Provide counselling and support to teenagers in a residential setting.
• Collaborate with external partners and stakeholders, ensuring effective communication and information sharing.
• Coordinate all external appointments for residents.
• Assess risks and needs of residents and families, developing Individual Care Plans (ICPs) accordingly.
• Work closely with care staff and programs team to implement ICPs and achieve resident goals.
• Conduct family counselling and facilitate reconciliation between residents and their families.
• Monitor daily progress and incidents involving residents.
• Contribute to Disciplinary Committee meetings and ensure follow-up on decisions.
• Maintain accurate and updated records for each resident.
• Conduct group sessions to equip residents and their families with essential life skills.
• Evaluate proposed external classes, workshops, and talks for resident relevance.
• Coordinate with agencies, internal teams, and the Programs department to organize sessions.
