Job Description


:

  • Conduct intake assessments and case conferences as part of multidisciplinary team when required
  • Part of multi-disciplinary team that develops and reviews clients ICP (Individual Care Plan)
  • Monitoring progress and outcomes of clients
  • Conduct Home/Hospital visits when necessary
  • Inform and assist clients in assistance schemes/ community resources that are relevant and current.
  • Any other tasks as assigned by Manager.
Requirement:
  • Graduate Diploma In Social Work/ Degree in Social Work/ Degree in Psychology
