Requirements
- Degree in Social Work or Psychology
- Willing to comply with the Standard Operating Procedure when discharging duties
- Willing to perform shift work in accordance with the Duty Roster
- At least 1 year of relevant experience
- Develop individualise care plans for residents
- Execute the care plan and monitor progress
- Develop and implement programmes and activities to meet the residents\xe2\x80\x99 care needs
- Put up 6th monthly review reports and monthly interview records for about 50 residents
Thuja Home at Pelangi Village for destitute persons is located at Buangkok Green.
Tembusu Active Ageing Centre is located at Eunos Crescent.
