• Assist with HSG assisted enrolment through small group sharing and one-to-one engagement with patients in polyclinic, via the mobile app or system application
• Explain to patients the benefits of HSG initiatives and the importance to enrol, make the first visit to review Health Plan with the Primary Care Provider, fulfil the care gaps of screenings and vaccinations and follow up for review annually
• Attend to HSG related enquiries from patients and caregivers
• Incentives management for patients who have completed their P4P and HSG screenings and vaccinations. Assist to conduct peer audit on incentives tracking at clinics

Requirement

• Min GCE O level & above
• Able to adapt, multi-task and work in a fast-paced and dynamic environment
• Customer service oriented
• Passionate working in healthcare industry
• Comfortable with the use of technology (e.g. mobile app and systems applications)
