Job Description
Our esteemed client is from hospital is looking for Dementia PractitionerResponsibilities:
- Work closely with dementia consultant to implement new care model
- Development of SOP for new care model
- Conduct staff training and provide support to staff
- Assist to review care model and fine-tune the care model with the feedbacks received
- Keep abreast with dementia care practices
- Any ad-hoc duties as signed by Head of Centre
- Degree in Counselling/Social Work/Nursing/Occupational Therapist/ Psychology
- At least 2 year of relevant working experience in dementia care
