Jobscope
- Perform nutrition screening with reliable screening tools to evaluate patient risks, level of criticality and to direct services while adhering to the Standards of Practice (SOP) in Nutrition Care.
- Conduct nutritional assessments with suitable methods which may include
- perform diet interviews to obtain a comprehensive diet history,
- conduct Subjective Global Assessment (SGA),
- participate in the interdisciplinary comprehensive assessments. * Integrate foundational dietetics knowledge with critical appraisal of assessment data to diagnose nutrition problem(s) which can be resolved or improved through treatment or nutrition intervention.
- Develop nutrition plans and implement interventions (e.g. diet counselling) based on knowledge of patient\'s current health and nutrition problem(s).
- Provide client follow-up and modify nutrition care plan as necessary.
- Involved in continuous planning, execution and improvement of nutrition projects and programs (e.g. outreach program, group education program, etc)
- Review literatures and/or utilize appropriate educational resources (e.g. workshop, conferences) to update knowledge and develop skill.
- Perform other duties as assigned.
- Bachelor or advance degree with major studies in food and nutrition or dietetics
- Completed clinical dietetics internship (at least 24 weeks) in approved institution; A member of Singapore Nutrition and Dietetic Association and obtained yearly certificate of Accredited Dietitian in Singapore (advisable)
