Responsibilities:
Experienced and with a proven track record of driving system level change, system integration and care transformation.
Provide the strategic directions and operationalization plans for the planning, development and delivery of integrated care and population health programmes.
Drive strong clinical partnerships and collaborations across key clinical committees and stakeholders to develop a shared framework, strategy and vision, and a workplan to develop clinical integrated care and population health programmes to improve the health and healthcare of our population in the Central North region.
Represent Company for national level meetings and workgroups pertaining to integrated care and population health.
Provide leadership to the Integrated Care Development team and across the various teams in GIC to strategise, plan for and implement clinical programmes that will advance clinical integration and population health for institutions and Central North region of Singapore.
Work with departments and teams in the ecosystem for programme operationalization, monitoring and evaluation for new and existing clinical programmes.
For clinical programme development, to take a strong support role to help clinical leads to develop programmes that are value-based, sustainable, evidence-based and with strong clinical outcomes \xe2\x80\x93 with the eventual aim of scaling them.
Perform horizon scanning of issues and challenges in the mid- to long-term timeframe and proactively work to surface these issues ahead of time to the team and senior leadership.
Requirements
Minimum Bachelor Degree in healthcare or relevant disciplines; advanced degrees in healthcare, public health, medicine or business administration will be an advantage.
Preferably with 10 years of relevant working experience in healthcare sector with at least 5 years of experience in healthcare leadership or clinical services development / integration roles.
Experienced and with a proven track record of driving system level change, system integration and care transformation.
Must possess strong team leadership, business administration, budget management and project coordination skills.
Experience with programme development and programme evaluation work will be an advantage
