Lead the team to provide effective and efficient operational support to the clinical departments under the department\'s charge
Lead the operational development, implementation and mainstreaming of clinical programs under the Division of Integrated and Continuing Care together with Medical and Nursing leads.
Work with external and internal partners to develop and support care transition services and programs between WH, primary care and the community care ecosystems.
Participate in the development of the integrated and transitional care strategy for Woodlands Health
Key Responsibilities and DutiesSpecific
Operational Support to the Clinical Departments. Lead the various operations teams under the direction of Director, Clinical Ops (Integrated Care) to:
Develop a deep understanding of the clinical department\'s patient profiles, care needs and care ecosystem.
Work with the medical and nursing leads to develop the clinical strategies to improve and deliver value-based care to patients
Help the department collaborate with agencies and community partners to deliver good health outcomes for its patients
Oversee the clinical department\'s workplan and performance indicators
Be part of the clinical department\'s meetings and help drive the agenda and discussion for relevant clinical and operational needs
Clinical Program Development and Implementation
Work closely with the medical and nursing teams to identify and develop the clinical programs.
Lead the operations team to develop and manage the projects within set budgets and timeline
Lead the operations team to develop the relevant policies, guidelines and manuals for the project
Regularly review and improve the clinical programs under the Division of Integrated and Continuing Care
Develop and support care transition services in WH
To support the overall planning and development of care transition for WH with the NHG Population Health Campus, primary care and community care.
To develop clinical and operational pathways that support patient flows between WH and the community
To collaborate with the Population Health Campus to harmonize clinical and operational models and minimize transitional care gaps in WH
Participate in the development of the integrated and transitional care strategy for WH
Conceptualise and organise regular partnership leadership, co-creation and networking platforms for multiple stakeholders.
Support the development of the integrated and transitional care strategy for WH
Be the secretariat for relevant steering committees and workgroups
General
Carry out any other duties as may be reasonably be required by the Head of Department
Job Requirements
Recognised University Degree, preferably in health and social sciences or business degrees. A Master\'s Degree in Public Health, Epidemiology or equivalent will be advantageous
Minimum 8 years of relevant experience in healthcare, community, or social care sector. Experience in community and transitional care, programme development & delivery, training and partnerships will be advantageous
Proficiency in Microsoft Office products with good written and verbal communication skills
Adaptable and goal-oriented leader who has strong leadership skills, is able to deliver under tight deadlines and in a high-pressure environment
Strong leadership, interpersonal and communication skills
Strong analytical and presentation skills
Good project management skills, with the ability to facilitate discussions and translate concepts into frameworks and care and operational models
