Job Description


The Executive, Community Care Operations will support the Manager to provide operations and administrative support to drive the population health agenda in the Central-North region of Singapore (NHG region) for the assigned operating unit(s)/area(s) of the department / Community Care division. Examples of operating unit(s)/area(s) include (but are not limited): (a) Care workstream / Sector-based programme development, (b) Zonal & Community Health Operations.The role will involve partners/stakeholder engagements (which could include government agencies, various multi-disciplinary teams (MDT) within Community Care Division and community partners across various health and social care settings) and to executive/implement services, programmes or initiatives within the Population Health Campus.Major Duties and Responsibilities (80%)

  • Perform required administrative tasks and support, co-ordinate and ensure efficient and smooth day-to-day operations of assigned operating unit(s)/area(s) and programme(s)/service(s).
  • Develop/review/revise workflows, processes, policies and all work documents e.g., guidelines, work instructions, forms for assigned programme(s) and ensure they are integrated, efficient and aligned with the relevant (clinical and non-clinical) governance framework/principles for Business-As-Usual operations and during critical incidents and/or national emergencies such as pandemic outbreaks, ensuring optimal operational efficiency and where required, implement strategies to improve service and quality standards.
  • To work with internal & external stakeholders/partners to execute/support the development of care models/flows, with a focus on resident/patient-centricity and continuity of care across different stages of a resident life journey and various care settings (e.g. hospital, primary care & community).
  • To work with clinical teams to provide secretariat and operational/project management support for various care workstreams (e.g. Frailty, Palliative Care, Preventive Care) or any project / initiatives (e.g. mental health or wellbeing programmes, programmes for low-income families etc.) under the Population Health Campus.
  • Establish and maintain partnerships with identified partners across the care continuum for scaling up efforts of community-based initiatives across Central-North Singapore.
  • To provide fund administrative support by managing financial resources, ensuring accurate allocation, tracking of funds across stakeholders and reporting of outcomes, in compliance with organisational policies and funding requirements.
  • Assist in financial budgeting exercises and support timely monitoring of key performance indicates and budget/resource utilisation.
  • To execute/support the development of enablers to drive efforts to improve capabilities of community care sector collaborators & share best practices and harness digital technologies or quality improvement methodologies to improve care integration.
  • To work with the Manager in monitoring identified social and health outcomes to inform service planning and performing periodic reviews of key indicators for programme evaluation and continuous improvement.
  • To support the Manager in keeping abreast of the local, national, and international population health landscape and best practices, conduct comprehensive literature reviews of relevant topics or analyse and understand the current care/resident journeys for different segments of the population. Where suitable, to recommend suitable strategies and programmes to the department and division leadership and identify gaps and opportunities related to the development of relevant programmes.

  • Builds and maintains harmonious working relationships and good teamwork among colleagues and external stakeholders.
  • Provide secretariat support for Population Health Campus and Community Care related meetings / training sessions. Duties include (but not limited to) scheduling and coordinating meetings, preparing meeting materials, minutes taking, circulation of agendas/reports/approved minutes and follow through of agreed action items.
  • Perform ad-hoc duties as and when assigned/required.
Job RequirementsEducation / Training / Experience
  • Possess a recognised degree in any discipline
  • Practical knowledge and experience in acute or intermediate and long-term care sectors will be an added advantage.

  • Possess a positive, proactive working attitude and able to multi task and manage various tasks/projects concurrently with specific timelines.
  • Strong project management and administrative skills also required, together with, good critical thinking and problem solving abilities.
  • Enjoy working in a fast paced and dynamic environment both independently and as a team player, yet is organised and meticulous.
  • Proficient in full suite of Microsoft Office tools - Microsoft Excel, Word, Powerpoint and Visio.
